A change which is always not in the direction of health positiveness. A negligible percentage of the rural population do continue to live in the traditional, slow paced, healthy lifestyle of our ancestors but a high percentage of urban (and in some cases rural) population have adopted a fast-paced, modern lifestyle in the pursuit of a %u201Cbetter living%u201D. Such a lifestyle has increased the consumption of junk or processed food, skipping of meals and sleep, lack of physical activity and a general apathy towards health. The increase in the consumption of fast food, too much dependency on simple carbs,high sugar and lack of fiber has spelled a devastating effect on the health of the society.     PCOS-What is it?

    Millions of women with PCOS are going undiagnosed, untreated and unsupported as the knowledge of the condition and its management most is necessary to overcome it in the first place. Are you one of them who is finding it difficult to lose weight or become pregnant?Having irregular periods or hair where you don't want it? Polycystic ovary syndrome (PCOS) is one of the common endocrine disorder in women of reproductive age of 18-45 years. This disorder is characterized by excess weight or resistant weight loss, irregular menstrual cycles, abnormal hair growth, acne, infertility, depression etc. Eighty percent of women with PCOS are overweight or obese. Research studies showed that 10% weight loss in PCOS improved menstrual regularity, fertility and other symptoms. Women with PCOS who wanted to conceive had better results with weight loss. Over the years, the lifestyle of our society has changed. The fast paced, modern lifestyles has also brought about a change in eating habits. The changes have directly impacted the health of society as obesity rates are sky high which is directly linked to an increased prevalence rate of PCOS. "Healthy eating is a way of life, so it's important to establish routines that are simple, realistically, and ultimately liveable" ( Arthur Agatston ) WHO GETS PCOS? About 18 per cent of women in India suffer from Polycystic Ovarian Syndrome (PCOS) and the increasing trend of PCOS is predominantly seen in the childbearing age group of 15 to 30 years. Further studies states that prevalence rate of PCOS is 3.7 % to 22.5 % with 9.13% to 36% in adolescents only.(Nidhi R, Padmalatha V, Nagarathna R, Amritanshu R. Prevalence of polycystic ovarian syndrome in Indian adolescents. J. Pediatr. Adolesc.Gynecol. 2011%u037E24:223227). WHY DOES IT OCCUR?   Answer of this particular question is still under research. Researchers and health care providers know that genetic and environmental factors contribute to the development of PCOS, but do not know exactly what causes PCOS. Other factors contributing to PCOS include obesity, dysregulation in ovarian function, abnormalities in the hypothalamic and pituitary gland function.(Legro RS, Strauss JF: Molecular progress in infertility: polycystic ovary syndrome. Fertil Steril. 2002, 78: 569576). INSULIN RESISTANCE AND LIFESTYLE Insulin resistance is one of the major contributors in around 50% to 80% of women with PCOS ,especially in those with more severe PCOS diagnosed on National Institutes of Health (NIH) criteria and in women who are overweight.     MANAGEMENT OF PCOS The overall management of PCOS should include both pharmacological and nonpharmacological management strategies. No two PCOS cases are similar. The management has to be based on individualized patients goals and symptoms like infertility, obesity, hirsutism or acne. Pharmacological or Medical Management For optimum results medical treatment should be used along with lifestyle modifications. Medical management can be broken down into classified as acute and chronic. Issues like irregular menstrual cycles, treatment of hirsutism and infertility management can come under acute category. Insulin resistance of PCOS need a life long management hence considered chronic issue. Non Pharmacological Management Lifestyle management with proper dietary approach and exercise is one of the important interventions in treatment of PCOS.In women with PCOS weight loss through dietary approach have been found to improve fertility rate,normalise excess angrogen levels and improve insulin sensitivity (Fertility Science and Research / JanJun 2014 / Vol 1 | Issue 1 29) There is no specific diet for PCOS but dietary principles should be focussed on low glycemic index foods with emphasis on moderate protein,high omega 3 fatty acids,Vitamin D supplementation along with proper management of bacterial flora in gut. %u201CA low carbohydrate,high protein diet helped insulin resistance. A high carbohydrate,low protein diet made insulin resistance worse.%u201D%u2013 Medical Journal Metabolism%u037E no. 12:14811487 There are some foods which really helps in making insulin sensitive and give overall support in improving complex symptoms of PCOS. Green Tea Flaxseeds Apple cider Vinegar Cinnamon Inositol Nuts Low Glycemic Index Foods Gluten Free and low diary products But all these varies based on symptoms,condition,age and purpose (whether to conceive,weight loss or any other reason) some foods are limited as they aggravate PCOS and may make the condition worse. These foods include: %u25CF Sugary beverages which are high in sugar, low in fibre, and can make your blood glucose levels soar. %u25CF Foods rich in refined sugar, fat, and fried food items. %u25CF We recommend to cut down all foods containing transfat, hydrogenated or partially hydrogenated food items like bakery products. %u25CF Always read labels and it is much better to avoid foods products that DO NOT come with labels (Local bakery products, local fried savouries or sweetmeats). %u25CF Eat more fresh and organic foods if possible. %u25CF Protect yourself from synthetic estrogen like plastics. Avoid using plastic in daily life as much as possible. %u25CF Use filtered water to avoid certain endocrine disruptors like lead. STRESS -Management...Why is it important? Stress seems to be a natural part of life with many of us feeling helpless in the crusade against it. Let us tell you how stress affects in PCOS? When you are in stress then our body pumps out more testosterone which make insulin less effective for your cells. This leads to weight gain, irregular cycles, depression, skin problem.  %u201CA report published in the Indian Journal of Endocrinology and Metabolism stated that %u201CStress can lead to changes in the serum level of many hormones including glucocorticoids, catecholamines, growth hormone and prolactin%u201D. Resistance Training : Generally people think that resistance training make them bulky or masculine but it is a mis concept.Running or walking will only help you to burn calories.To recover ormonal imbalance there should be a perfect combination of cardio and resistance training with proper support of diet.Different research shows that intense cardio exercise generally imcreases cortisol level which causes adverse effect on those who are sufferering from PCOS.But if we combine cardio with good resistance training then it balances cortisol level. If you are not sleeping and leading a stressful lifestyle-will tend you to gain weight. We should not wait when hunger strikes as it kills our metabolism,one of the contributory factors in gaining weight.Better we should adopt habits of eating healthy 5-6 meals, every 2-3 hours of interval and meals should always be rich in protein,fiber with a healthy combination of complex carbs,vitamins and minerals. If you wait for the next hunger bells then you will end up eating all the foods insight and not able to think of healthy meals rather will end up eating sweets,junks and salty items. Wake up early in the morning at 6 am or even before. 2. Eat your first meal within 30 min -1 hour of waking up. 3. Start your day with a whole fruit but not with tea or coffee. 4. Workout for 30-45 min everyday but do not forget to take a pre-work out snack like fistful of nuts/cup of green tea/fruit. 5. Take your breakfast within 8 AM but make sure you have good quality protein in your plate.Do not delay breakfast later than 10 AM 6. Ideal time for Lunch is 12.45 PM and maintain 4 hours gap between your breakfast & Lunch. 7. Ideal time to take dinner is between 7-8 PM but should not cross beyond 10 PM. 8. There should be 2-3 hours gap in between your dinner time and sleeping time. 9. Eat mindfully and enjoy each and every bite of your food.Do not forget to chew it well. 10. Sleep for 7-8 hours for a positive and healthy start for the next day...
